The National Center on Physical Activity and Disability (NCPAD) is an information center concerned with providing people with disabilities a variety of resources to help them lead healthy and physically active lives. NCPAD and the National Center on Accessibility (NCA) are recruiting parks and recreation areas to provide information about their accessible recreational opportunities by completing the NCPAD Park Survey. The purpose of this survey is to build a public database of accessible park and recreation areas for people with disabilities seeking accessible recreational opportunities. The database is maintained by NCPAD at the University of Illinois at Chicago.
Participation in this survey is voluntary. The survey will take approximately 10 minutes to complete. Please fill in all items to the best of your knowledge. If you have any questions regarding this survey, please contact ncpad@uic.edu or call our toll-free number at 800-900-8086.
