National Center on Physical Activity and Disability
NCPAD is an information center concerned with physical activity and disability. Being physically active is good for
every body. That's a message you will find many times on our site. Being active is an important part of getting and staying healthy, and we offer many ways and alternatives for parents and their children to get involved. At times, parents may have a difficult time finding welcoming, beneficial, and fun programs and activities for their children with disabilities to participate in within their communities. We help to make this process easier by providing a vast amount of helpful and informative tips, resources, and forums that are available to assist parents in facilitating and supporting an active lifestyle for a child with a disability. You will find suggestions and ideas to assist you and families in helping select activities, set goals, and promote independence, to only name a few. Working with Illinois Department of Transportation

As many of you know, we are currently working with the Illinois Department of Transportation on the Safe Routes to School Program to create curriculum to promote and maintain the participation of children with disabilities in walking, wheeling, or biking to school. We find school, teacher, parent and community feedback essential in this project and all that we work on. Our goal is to begin a head count of the number of children with disabilities who are participating in the Safe Routes to School Program. Once we have a baseline number, we will create curriculum and informative materials to further promote the participation of children with disabilities. In order to do so, we have developed both a Teacher and a Parent Survey that will be used to collect data on the number of children with disabilities participating in the SRTS program in Illinois.
The links for these surveys, which include an
English version of the Parent and the Teacher Survey, are located on this page. We also have a
Spanish version of the Parent Survey. The teacher surveys are broken into three categories: Administrative level, Grade level, and Classroom. The school should decide the level of surveys that will be used. Each student should only be accounted for in one survey. If you would like to have one person complete the survey for the entire school, choose the
Administrative level survey. If you would like to have someone complete the survey for each grade level, choose the
Grade level survey and complete one for each participating grade level. For example, if you have six grade levels participating in the program then you will complete six separate Grade level surveys, one for each participating Grade level. Likewise, if you choose to do the
Classroom level survey, each teacher will complete the survey for their own classroom. If you have trouble viewing the surveys, please contact us.
We strongly encourage all teachers to complete the online versions of this survey. Should you wish to distribute the online version to your parents, we can provide wording that can be copied and sent out in email format. Please contact us to receive this text.
Should you choose to complete a paper copy of the Teacher and Parent Surveys, please contact us with the number needed (English/Spanish) and address for shipping.
If your program/project is year long, feel free to survey at any point during the school year. Please note that your contracts with IDOT state that you are obligated to complete the survey at least once a year. Your participation is essential and our work to include children with disabilities in all SRTS programming cannot be accomplished without your feedback.

We would like to thank those of you who we have received responses from in the previous funding cycle regarding the Safe Routes to School Program and the participation of children with disabilities. Please note that we have changed the survey in response to your feedback and welcome your comments, again, on how we can make this easier for you. We welcome those of you new to this process and ask that all grant recipients funded in the current cycle complete this important online survey. We greatly appreciate your time and consideration. The information that you provide is key to the inclusion of children with disabilities in SRTS programming.
